Admins:
I am running an Alphaserver 4100 that is crashing intermittently for an
unknown reason. When the system crashes, -everything- is disabled, even
video output. I'm positive that the cause of the problem is a hardware
component, but I'm not certain which. The system is able to boot up to the
SRM (three-character) console. I have run 'test', 'test mem', 'test cpu',
and 'test pci'. The memory, the CPUs, and the power supplies are reporting
that they are fine. When I test the PCI devices, however, I am receiving
this error:

Initially, I thought that this error was being produced because there was no
floppy disk in the drive. Putting a floppy disk in the drive and re-running
'test pci' does not get rid of the error message.
I have searched both the Alphaserver 4100 documentation I have here, and
I've searched the Compaq documentation online, and I have not found any
reference to the error code above. Does anyone have any idea where this
error message is coming from, if it is actually referring to the floppy
drive, and if it could be related to the catastrophic system crashes that
this system is having on a daily basis?
This is probably unrelated, but this server is running with 4 CPUs and 2
power supplies. There is room for a third power supply. The system
recognizes that one is missing, but does not appear to have any issue with
it.
