Hi all,
We've recently made a failed attempt at upgrading one of our 8400s from
4.0f PK4 to 5.0A on our way to 5.1. There's nothing particularly unusual
with our configuration (that I'm aware of), but it does use LSM mirroring
on the root and usr partitions.
The log of the end of the upgrade follows:

As you can imagine, this left the machine in a pretty much unusable state.
I've looked through the /updmnt/isl/setld script (from the v5.0A OS CD)
and I can't work out why it couldn't find "tail" in the first place. My
path was fine:
# echo $PATH
/sbin:/usr/sbin:/usr/bin:/usr/ccs/bin:/usr/bin/X11:/usr/local
#
and I had /usr mounted. The only thing I can think of is that for some
reason "tail" and other commands got removed before the upgrade.
Has anyone seen anything similar before or have any ideas where things may
have gone wrong. In any event, I think next time we'll try for the 4.0f
-> 4.0g -> 5.1.
