I had a problem with an upgrade on an 8400 from 4.0F -> 5.0A see

Thanks to Dr. Thomas Blinn, Jonathan Williams and Pat O'Brien for their
responses.
Jonathan and Pat reported seeing the same problem during the same upgrade
and ended up doing fresh installs.
Looking back through the logs of the upgrade, one thing that I didn't
follow by the book for the upgrade install was the method of getting to
single user. Instead of going from multiuser to single user and doing a
bcheckrc (which is the way recommended by the book and Dr. Tom), I booted
up from then console (I wanted to capture the console variables) to single
user and did a bcheckrc. I've just had a quick read through the
Installation Guide for v5.0A and found the following:

Caution
If your system is at the console mode prompt ( >>> ), you must boot
to multiuser mode and then shut down to single-user mode. It is important
that you do not boot the system to single-user mode.

This caution doesn't seem to appear in the 4.0x guides.
I've tested this theory by doing the 4.0F -> 5.0A after booting to single
user from the console on a PWS433au (although on a very bare-bones
install) and it all goes flawlessly. I guess it's possible there are
issues with regard to LSM and other layered products we had installed on
the 8400.
I guess the upshot is, always check the new installation manuals for new
info and follow it to the letter. Then if things go wrong you may be
able to blame someone else...
