video card support

I have installed 4.0D on an Alpha1000 system, when I added a DEC PowerStorm
PBXGB-AA video card and set the console to graphics the system boots, but
when it should load the CDE the screen goes blank and the signal to the
monitor is lost.

The only information that I can find says that switch SW1 should be to 0 and
that Alias and VGAEN should be set on the card...

Any suggestions?
