As I had suspected this was a console version issue. The firmware version
was several versions ahead of what V5.0 originally shipped with.

As Dr Thomas P. Blinn from COMPAQ said:

The firmware grew in later versions, and there was work done in the
kernel build procedures to be able to deal with it; I suspect that
you have newer firmware that needs the new kernel features that were
not in the old release.

Edward Ady from COMPAQ provided the solution:

There is a console variable that you can set to alleviate this problem:

>>> set console_memory_allocation old
Power cycle machine.

Which fixed the problem.
