OSF 4.0F Patch kit 6 issues ?

I vague remember issues with patch kit, but can not find data. All I can
find so far is issue with the binary.errlog corrupting itself. I have
shutdown binlog and syslog, cp dev/null and restarted to no avail. I then
stoped them again wiped out the file, rebooted still to no avail. Any
ideas, and/or refreshes to patch kit 6 issues?
