Thanks to Scott Rogers, Daniel Monjar, Pat O'Brien and Antonio Gonzales for
their resposes. Antonio's response did the trick:
Run the EISA Configuration Utility with the ECU floppy. Once you touch the
SRM firmware, you "have" to run also ECU
(just go to ARC console, and run ECU from floppy, browse you config and save
& close; change console back to OpenVMS/UNIX; power cycle; and try again).

I have a alpha 4000, 640 MB of memory, 1 cpu 5/300. I am trying to
install 5.1A UNIX and when trying to boot from the CD-ROM, I take a kernel
memory fault. I tried this with 6.0 version of the firmware and I also
tried with 5.7 of the firmware and the same thing happend. I tried booting
a 5.1 disk and also took a kernel memory fault. Any Ideas?
