How to create fail-safe diskette

Hello tru64-unix-managers,

  We have ALPHA 1000. I want to create Fail-Safe diskette, but i don't
  know how. I can find documentation about that: How to use Fail-Safe
  Loader (FSL) but I don't know how to create the disk. Any ideas.
