Voliod "illegal vminor encountered" errors preventing volumes from mounting

All,
A company I am working with is running Tru64 V5.1 on DEC 4100's, A1000's,
GS40's, & GS60's . We loaded level 5 patches using dupatch and received
the error "Aug 22 11:36:38 iadd005 vmunix: lsm:volio: Illegal vminor
encountered" on bootup ever since. This prevents lsm from mounting most
of the volumes on the system. System investigation shows Vold never starts
as a result of the error. We found a work around by mounting the volumes
manually. Here is the cmd sequence that seems to work:

vold -k
volume startall
mount -a ( all of the mounts are found in fstab)



We backed out from patch 5 to patch 4 and are still experiencing the mount
problems. Does anyone know of any bugs with patches, lsm etc. that would
cause the behavior. Any suggestions to troubleshoot and correct the
problem? I have included my messages file for reference. Any help is
greatly appreciated.
