Big thanks to everyone who responded on this one!

The problem is a failed power supply, as suspected. The lower p/s
has no lower green light on, which shows that there is no DC output.
However, there is a workaround that may get this machine back in service.

My machine has four CPUs and therefore is required to have two power
supplies functioning. There is a jumper J3, located on the IO board (the
card between the eisa and pci slots) that sets up the machine for dual
redundant power if connected. The jumper was on; however, that was the
incorrect setting for this configuration. It should have been "off" with
four CPUs installed, not that it would have made any difference in this
case.

The solution was simply to take out two of the CPU cards. I did
that and was able to get it up to an SRM prompt.

It looks like power problems are not logged on this model by Tru64.
