Problem with scanned in devices

The system is GS140, the storagework is hsz70, with scsi connection.

The system is running 5.1b and hsz70 controller is v77z-2. Console firmware is V6.3.1

I installed a mirror set in the hsz without any problem. In the system, I tried to use hwmgr -scan scsi to scan in the devices. Although I wont' see anything when running hwmgr -view device yet I can see it in hwmgr -show scis with (null) device file. (see the end of below output with hwid as 145 and scsi device id as 14)

While in the console, there is the messages of:

dsfmgr -k won't help either, and dsfmgr -vV shows everything is fine.

I suspect that something left from a removed device dsk11 caused the problem and I first used dsfmgr -D dsk11 which didn't work and I deleted all the dsk11 files in /dev/disk and /dev/rdisk but it still won't work.

Finally, I had to install another mirrored set and it was scanned in without problem, which is dsk44.

What is the cause of the problem and is there a way to solve it?
