UPDATE: sendmail wrappers.
It seems that some of the info I gave in my summary is incorrect.
As Keith S McCabe noted:
"Although tcp_wrapper is a *great* piece of software it
is not well suited to mail delivery filtering.
If you address mail to me as Keith.McCabe%ranplc.co.uk_at_ftp.uu.net the
tcp source address will be that of the ftp.uu.net mailhost and not your
own mail relay's tcp address as mail will be redirected to ftp.uu.net
first. "
Another error was noted by Gyula Szokoly.
(this is what I wrote first, and then his reply)

NO! Sendmail is designed to run as a daemon. If you have it
started by inetd, you can easily kill the machine by a heavy mail load. On
a client, it's OK, but I wouldn't try this on a mail hub....
The information on procmail seems to be OK.
Everyone who answered seems to agree that procmail is the best mail filter.
