Tape drive errors

Can anyone speculate as to why I would be getting I/O errors when
backing up to tape?

I am using tar to backup my web server directory and it starts out fine,
but then partway through, I get this:

tar: write : I/O error
tar: Tape write error
 
When I do 'tar tvf /dev/rmt0h' it says this:

tar: [offset 0]: I/O error

Is this a problem with tar, the tape drive, or the tape? Any pointers
would be helpful. Also, is there any on-line information concerning
alpha administration? I have been managing some services on Alphas, but
now I am responsible for the whole show and don't have much DU
experience.
