User quotas not working?

Suddenly it seems that the user quotas on my AdvFS filesystem aren't
working...

I have the following fstab entry:

acc_dmn#acc_fs /usr/acc advfs rw,userquota

quota.user and quota.group also exist, and quotaon -a has been run. However,
users who have quotas are not prevented from exceeding them, or even warned
about it.

I didn't change anything with regards to this, and it seems to have happened
recently...anybody have any ideas?
