warning on AlphaStation 255: firmware update needed for DU 4.0

If any of you are considering updating your AlphaStation 255 to Digital
UNIX 4.0, be aware that you do need to update the system firmware.

It's not obvious that you need to do so, since if you look at your
system boot logs under 3.2D you see "Firmware revision 6.0" and the
minimum required firmware level listed for DU 4.0 is 6.0.
But the OSF PALcode version is newer on the CD-ROM shipped with DU 4.0
(firmware update CD-ROM v3.5), and you will need the updated PALcode to
boot the GENERIC kernel halfway through the update process.

To anybody inside DEC: take this as a request to have the chart in the
firmware update release notes include the required PALcode revisions as
well as firmware revisions.
