Thanks to Sam Sarasin, Stuart Davidson, Pirie Hart, and Tom Reu for
answers to my query. Pirie Hart gave me the information I needed:

I've still got a problem booting over the net, which I will detail in
a separate piece of mail (for filing convenience :-).

  (The following applies to DU 3.2C. Your mileage may vary.)

[But actually it worked for 4.0A too. -j.]

  The directory "/usr/.smdb." contains the subset files. Files with the
  ".lk" extension indicates the that the subset corresponding to the name
  of the file, with the extension removed, is installed. The files with
  ".ctrl", ".inv", and ".scp" should be present for all subsets which you
  have had the option to install.

  The ".inv" file lists the files and directories that are included with
  the subset. On my system, OSFRIS350.inv includes a line for your missing
  file "/usr/var/adm/ris/bin/getname".

  You should search all "*.inv" files for a reference to the appropriate
  "getname" (i.e., "grep getname *.inv"). You may find several files
  contain "getname". Once you have found the correct subset, reinstall it
  using setld.
