AlphaStation 600 5/266 Panic "pci_map_unload - no pte?"

Hi Managers,

Does anybody saw this panic before?
We have an AlphaStation 600 5/266 runing Digital Unix V4.0 (rev 386).
I've check the Patches, but found nothing.
