vdump Failure After 4.0B Patch Kit #4

We're running DUNIX 4.0B on a AlphaStation 200. I recently installed
patch kit number four from ftp://ftp.service.digital.com/, and now I'm
receiving the following error when using vdump:

path : /users
dev/fset : users_dmn#users_fs
type : advfs
advfs id : 0x319b7053.00092e01.1
vdump: Date of last level 0 dump: the start of the epoch
vdump: Dumping directories
DECthreads bugcheck (version V3.13-439), terminating execution.
mutex 0x3ffc0082740 ref. overflow on unlock (0x310000:0xdbcafe1:0x2:0x14001a210)

Anyone else seen this problem? If yes, how did you fix it?

I reviewed the archives and found some problems with vdump but it was
related to the JDK 1.1. Two patches which were installed as part of
patch kits are:

OSFPAT00001000410 installed Patch: dump and rdump Command Corrections(Commands, Shells, & Utility Patches)
OSFPAT00009600410 installed Patch: Threads Corrections (Library Patches)

I thought I might remove the above threads patch, but I thought I seek
help here first.
