In what configuration must new memory be installed?

Janusz Mrozek, Tom Blinn, Lew Lasser, Grant Ward, and Steve McLaughlin,
all offered the similar information. Their explanations were clear, and
yet it took several readings (and a few more failed attempts) to resolve
the issue. The bottom line is this:

The motherboard has four slots for daughter cards/carriers. Each
daughter card holds 4 SIMMS: 2 on "top" and 2 on "bottom". The "top" row
across all 4 daughter boards comprise one bank of 8, as do the "bottom".

CARD 1 CARD 2
---------------------------- ----------------------------
|2 ========= 2 ======== | |2 ========= 2 ======== |
| | | |
|1 ========= 1 ======== | |1 ========= 1 ======== |
---------------------------- ----------------------------

CARD 3 CARD 4
---------------------------- ----------------------------
|2 ========= 2 ======== | |2 ========= 2 ======== |
| | | |
|1 ========= 1 ======== | |1 ========= 1 ======== |
---------------------------- ----------------------------

Intuitively (at least for me) it would seem that a bank some combination
of 2 daughter cards would comprise a single bank. As you can see above,
this is not the case - each bank spans all 4 boards. At that point
everything worked as expected.
