DPW console firmware refuses to recognise QLC ISP1040 - iteration 2

Hi everybody,

Some time ago, I sent out a question concerniog rev. G of QLA1040
SCSI adapter being not recognized by console firmware in au-series
DPWs. It is recognized, but not initialized due to the lack of
firmware by DU 4.0d. At the same time, the Digital KZPBA (which
in our case appears to be an unmodified[*] rev E. of QLA1040)
works fine.

I got no useful replies to that request, which lead me to investigate
a bit further. Disassembling the DU's Qlogic ISP driver (which is in
/sys/BINARY/isp.mod) reveals that it contains the code to reload
firmware of the Qlogic 1020/1040 adapters[**]. The firmware itself is
supposed to be in /dev/qlogic/isp_fw.mod. Unfortunately, my skill at reading
alpha machine code is not sufficient to figure out that the format of
this file is supposed to be[+]. Placing binary image of the firmware[++]
in /dev/qlogic/isp_fw.mod does not work - the driver attempts to get
the firmware, but aborts with: "Unsupported firmware image file format".
(The message appears to come from the driver's routine responsible
for loading firmware image into memory. It does not attempt to download
it to the card itself).

This brings me to yet another question: does anybody here know that
the format of /dev/qlogic/isp_fw.mod is supposed to be, or better
still can share his/her copy of it with us?

[**] Which means that you still can't boot from it. That's fine
for us - we want to use this card alongside the KZPBA.

[+] Naturally (well, for Digital), the format (or existence) of
/dev/qlogic/isp_fw.mod does not appear to be documented anywhere.
I also can't find any trace of it by grepping through the entire
contents of our Firmware and DU 4.0d CDs.

[++] included with the Linux-Alpha ISP driver, and also easy to
extract from the QLOGIC's x86 boot PROM on the card itself.
