The procedure is:
>From the SRM prompt, using either iconfig or isacfg, create an entry for
the ISA device with the appropriate DEVICE NAME for the DU version being
used. Those are:
ISA-VGA for v3.x;
MACH64 for v4.x,
and mark it as ENABLED. The slot number used is irrelevant;
Ultimately, my problem, after switching the device name to MACH64, was
that the video cable had a broken wire inside. It would work when
plugged into the PCI graphics card, but not when it was rotated and
plugged into the ISA graphics card. After I replaced the cable, it
worked like a charm.

I have an AlphaServer 300 4/266 running 4.0D, SRM
console V6.6-1.
It currently has 2 PCI cards (max # supported), a DE500
NIC and a S3 TRIO 64 Graphics. I need to free up one of the PCI slots
for use with a KZPSA controller, so I obtained an ATI Mach64 ISA
graphics card to place into the ISA only expansion slot. Digital Part
number for the Mach64 is PB2GA-FB and it is supported on this platform
according to the supported options list on the web.
I don't know how to configure the ISA slot to activate
the card. The AS300 User Guide says to use the SRM script add_mach64
but that script did not configure the ISA config values. I need the
proper values to enter into the SRM iconfig utility (or the isacfg
utility), such as I/O, IRQ, DMA, BaseAddr, and Length.
Does anyone have the appropriate values? Or, if I'm
totally off-base, point me in the right direction.
