Multiple instances of libc.so and libc_r.so

Recently applied some patches for DU-4.0b resulted in
the following error messages:

28837:./cpl: /sbin/loader: Fatal Error: Multiple instances of shared
object libc.so loaded as both /usr/shlib/libc_r.so and /usr/shlib/libc.so

For the patch, we rename (mv) the old libc* and cp the new stuff
over. Rebooting doesn't seem to help. Restoring the old libc*
doesn't help either. Any suggestion as to how I can resolve this.
By the way, this error message seem to only show up on a few application.
