ATMworks 351 network card not initializing on boot

I'm trying to add an ATMworks 351 network card to an AlphaStation 255 4/233,
but at boot I'm getting the message "Couldn't register with CMM".
Specifically, the bootup sequence looks like:

  lpa0: ASIC Rev: 1, PHY type is MMF
  lpa0: Mapping registers to memory space 0x82048000
  lpa0 at pci0 slot 11
  lpa0: Total 16 MAC addresses:
  lpa0: Digital DGLPA ATM Interface, hardware addresses:

  lpa0: couldn't register with CMM

The device does show up in the console mode as "Vendor: 102F Device:20", as
the README file says it should. The README file entry for the error above
says:

  This is a serious error, the driver will not run correctly.
  Contact your support person or update the driver.

To resolve this, I've tried (in order):

 1. Moving it to a different PCI slot. The PCI slot it is in now is the
    lowest one...the combination PCI/ISA slot which shares interrupts with
    the embedded Ethernet controller. There was a note distributed with
    the machine that said some drivers don't have shared interrupt support
    (the AV321-AA card is one in the machine that has this problem). Moving
    the ATMworks card to the middle slot had no effect.

 2. Updating the firmware to the latest available -- version 6.6 (was
    version 6.2)

 3. Installing the latest aggregate patch set from DEC for DUNIX 4.0a
    (dated 19971216)

 4. Removing the LPA100 saveset, redownloading it off the net, and
    reinstalling it.

DEC's hardware support suggested updating the firmware and aggregate patch
installation. I haven't been in touch with software support yet (local
problems with tracking down the support contract number).

We are trying to connect to a FORE ATM switch over multimode fiber.

If anyone has any suggestions, I'd appreciate hearing them. Thanks in
