I'm still not sure exactly what happened, but when a user accidentally
got themselves totally locked out (too many attempts) I stumbled onto
how to fix the problem. I went into dxaccounts and manually locked,
then manually unlocked the account. Voila, logins succeed on the first
try. I repeated the process on some of the user accounts, and it fixed
those logins as well. So it appears that somewhere something was
corrupted in the tcb files.
