The problem in my case was that there was a default ACL (displayed by the
getacl -d command) on the user's home directory. When I removed the
default ACL, the files were created with the correct permissions. The
default ACL overrides the umask setting.
Another consideration that did not apply to me was:
If the home directory is remotely mounted, that could be the cause.
