DU 4.0D 8400 crashes with bs_map_bf

Hello Everybody

I have a DU 4.0D system that has crashed 4 times in 24 hours with :-

04-Feb-1999 00:39:25 panic (cpu 1): bs_map_bf: required volume not
available

Has anybody seen this before or know what causes it or how to fix it?
