Problem with root file System space

Hi,...
We have an alphaserver 800 running Digital Unix 4.0D, yesterday happened
us a queer thing with the root files system, the space available for the
root files system went gradually decreasing , we tried to find a
background process that would be writing something to the root file
system using the fuser command, and others commands to monitoring the
space (du, df , etc...) , but any of the process seemed to be writing
some. Other thing, when the system reach the 100% of space used, we
halted the system and then in the init appeared an strange error:
        vm_swap_init: vm_swapon for /dev/rz0b device failed
        vm_swap_init: swap is set to lazy (Over commitment mode ) ....
something like that..

I don't know if Is there someone that had had this trouble ?... I'd
