Below is the "byte" trail.....
Thanks to those who responded and pointed out the "patch" scenarios
For both oracle and compaq

Have a GS140 with Oracle.....Scenario as follows
in START.log...

"After DB starts up"

Database "VistaXXX" warm started.
Internal AIO consistency error: Cannot initialize aio library.
       Error 35: Operation would block
       Aborting ....

" Error repeated from line 672 of file to 2804 EOF"

ANY help would be appreciated and will (individually)
provide any more information that would be helpful
to a solution.

 You need the aio library patch from the digital v4.0x patch web site.

We had this problem, here's info I had:

Thereis'a bug with oracle 8.0.4 and DU4.0D
        - thread safety issue with library-raw - laio_raw
        - patch in 8.0.5
        - Internal aio inconsistancy
        - If you can't upgrade, fix is:
                - relink oracle, removing references to libaio_raw

Dec knew about it, provided me with info. Folks here did the
modifications
to oracle and relinked, it worked.

Hi Mike !
 I think there is patch for this problem at digital unix site.
 Either if you contact oracle support they will tell one more
 workaround.
 ssm-threhold parameter value should be zero to avoid this problem.
 Please check digital web site for the patch.

WE ARE RUNNING 4.0 E     
loaded at install 
and we loaded the only (known) patch for 4.0 E
back on march 4th.
Will the old (rev) patch work on 4.0E ????

Patch was included in [oracle] 8.0.5. We were not ready to upgrade, so
we rebuilt oracle 8.0.4. We are at DU4.0D, but I don't think that was
an issue.

Thread-safe libaio_raw patch didn't make it in 4.0-E patchkit 1. 4.0-D
patch 
is good on 4.0-E as well. Also, there's one other bugfix in 8.0.5.1 in
this 
regard (which might generate this error even after you install the 
thread-safety patch). Don't remember bug# off the top of my head.
