Luc Patoine from Compaq/Digital customer support found our problem:

My initial problem was that after upgrading to 4.0d I had a 4100 machine that
crash upon booting and
another one was not seeing some scsi devices. Both machine would crash when
trying
to boot from the CD 4.0D but were ok with 4.0B
SO before upgrading to 4.0d verify that you dont have SCSI id higher than 7
defined on our HSZ70 controller,
because you'll have some bizzarre problems.
