I got the help from Alan (see his email included here), applied solution
#1, and everything is beautiful again.
(Alan did not tell me his full name nor where he works, but from the way
he knows the program, I guess that he is Mr. Alan Rollow - the author of
our beloved "monitor". Thank you Alan, for the help and for the program
"monitor".)
Here is the email from Alan:
Solution #1. Remove -DNEXUS_LIST from the list of makefile
defined constants, recompile disk.c and relink.
Solution #2. Give permission for me to mail you the current
version that fixes this a few other problems. If you select
this option decide whether you want the whole distribution
in a single uuencoded compressed tar archive, the encoded
compressed archive of just the sources or individual source
files.
Solution #3. Track down the latest freeware CDROM and get
the just previous to current version that does fix this
problem, but doesn't have all the fixes in it.

Hello Guru,
We have an Alpha 4100 running Digital Unix 4.0D. Last week, we added a
KZPSA FWD-SCSI host adapter to connect to an external disk array from
Open Storage Solutions (a differential device). Then, we rebuilt the
kernel, set up the disk array, etc. Now everything is working fine ....
except our beloved freeware 'monitor', which now gives us a coredump and
refuses to run. We even recompiled the program 'monitor', but the
problem still persists. Do you have any clue?
Note: The 'monitor' program was downloaded from
ftp://gatekeeper.dec.com:/pub/DEC/
