Who both provided solutions to my problem, which is now fixed. The problem was
| When we do a full boot from the console i.e boot_osflags=a
| the system boots into
| the following state
|
| Login banner. Instead of the usual we get
|
| Digital UNIX 4.0 (Amnesiac) console
| login:
|
| No addition swap space is added ( only default available)
| No licences are loaded
| Can't init3 the machine.
| None of the startup scripts are run. i.e /etc/rc*d
The solutions are

I've had this before; if you get corruption in root_domain using AdvFS,
you get this sort of corruption; nothing loads, hostanme reverts to
Amnesiac
and a whole swag of problems.
If you boot in single user and do a verify -r root_domain you should see a
whole
bunch of errors.
Best fix: get a backup of root_domain, boot off CD; blow away root_domain
(completely
delete the filedomain; just removing the fileset isn't enough), recreate
the
domain
and fileset again and restore.

It was due to files being located beneath my mounted filesystem. Once I
unmounted everything, cd'd to the mount point, removed the files, and
booted back up, we were good to go.
It seems that the machine has been patched without the /var filesystem mounted.
This process created a /var directory containing files and directories
which was actually in the root filesystem and beneath the /var mount point.
These were moved and the machine booted as it should.
