Thanks to all who replied.
Not surprising the answer to the my original question (which firmware
version is needed for 4.0F) was that only the latest version (5.4) was
supported (but older would probably work). But since I managed in the
meantime to dig up a firmware-CD I installed it and then did the upgrade
and everything works fine now.
