[Q] panic (cpu 0): blkfree: freeing free block

Just had a system (4.0E) crash with these two message:

   vmunix: dev = 0x808406, block = 11339, fs = /usr/users
   vmunix: panic (cpu 0): blkfree: freeing free block

There was a lot of activity on the /usr/users partition (UFS)
at the time.

The machine boots and everything looks good. What I would
like to know is does this indicate a hardware problem with
the disk or is it a software problem?
