XP1000, powerstorm 350, CDE problem

I've got a XP1000 that doesn't get the CDE window. When booting, after the
starting dec windows messages appears, the screen goes black and the hour
glass is displayed. The box for the console log is drawn but no text or
scroll bars appear. The console/mouse/keyboard freezes in this state.
Sometimes I can remotely telnet in, sometimes not. I can ping the system
and get a response, try to telnet and the system hangs and then try to ping
again with no response. If I boot to genvmunix (vga, low resolution) the
CDE window is displayed and the keyboard/mouse/console functions and I can
log in with no problems.

The XP1000 is running 4.0f, firmware V5.4-9465, PALcode 1.47-17, with a
powerstorm 350 video card in the third slot. The powerstorm drivers have
been downloaded and installed. In the /var/adm/messages the following is
displayed:
per0 at pci0 slot 13, kernel console: per0. The system is not running NIS
and portmap is running. The domain name and servers have been defined.
Open3D license has been installed. The video card has been replaced. The
system (4.0f), Open3d and the powerstorm drivers have been reinstalled
with no effect.
Any ideas?
