exception system internal error

I've been trying to increase program size limits for one of our users.
He's using 3.2B (yes, I know it's old, not y2k-compliant, etc. - you
don't need to remind me). I've (1) increased swap space, (2) doubled
maxdsiz (from the default) and rebooted and (3) doubled maxvas (from
the default) and rebooted. I also removed the /sbin/swapdefault link
to switch from immediate to deferred swapping, but haven't rebooted
yet. These changes may (or may not) be related to the following
problem.

The user now says he has a "small" program that, immediately upon
starting, causes the disk to start "chattering" and a message appears
which says: "exception system: exiting due to internal error: out of
memory trying to allocate exception system source." The chattering
continues until he hits ^C. He says he hasn't changed the program
since the last time it ran okay.

So what does that message mean? And how do I get rid of it?
