Problems using ADVFS through itpsa driver

  I have a Compaq XP-1000 running Tru64 UNIX 5.0 with an Intraserver 6201U2
dual-channel ultra2 SCSI interface card connected. I'm having problems
creating an advfs file system on a disk connected to the machine through
this interface card. The disk (a Seagate Barracuda 50) formats and operates
okay as a ufs file system, but if I try to format it as an advfs system
using mkfdmn, I get a large number of errors in the system error log and a
mkfdmn error:

mkfdmn: domain init error I/O error

An example error that was e-mailed back to me by the error logger was:

  ============================ Translation =============================
Sequence number of error: -839385038
Time of error entry: 10-Dec-1999 13:40:31
Host name: rossby

SCSI CAM ERROR PACKET
Controller type: DISK
SCSI device class: DEC SIM
Bus Number: 4
Target number: 0
Lun Number: 0

Name of routine that logged the event: ss_perform_timeout
Event information: timeout on disconnected request

                ############### Entry End ###############

Event information: Active CCB at time of error

                ############### Entry End ###############

The problem isn't unique to this disk. I ran into the same difficulty
trying to format as advfs on older Barracuda 18's through this interface.
The disks work fine as advfs when mounted internally.
I have the latest patch kit for v5.0 installed. Is anyone successfully
using advfs through an Intraserver card (itpsa driver)? Are there known
problems with this driver under v5.0?
