/usr partition at 119% on 4.0d

We are running an 8400 with 4.0d and the /usr partition is at 119%, but
when we look at showfdmn we find the partition is only 89%. can anyone
offer some suggestions? we are getting error messages about filesystem
full, and unable to write to it. We are not running quota on our
system.
