Problem with Mirroring of Plexes at LSM

hi gurus,

Environment:
we have a Trucluster (PS) cluster of two 4100s. Most of the disks are
on the shared SCSI bus except for /, /usr, /var, and swap, which as
Trucluster requires are mounted locally and mirrored through LSM.

There are many Disk Grups. Under each disk group there is one volume.
One of the Volume has two Plexes(Mirrored).

Problem:
one of the Plex which was originally part of the Volume has come out of
the volume.

action taken:
tried to drag and drop through the dxlsm screen.

error:
'volplex -g <diskgroup name> att <plexname> -02 error'

fsgen/volplex: volume <volume name>, plex <plex name>, block
       121472: Plex write:
        error: Write failure
fsgen/volplex: I/O error on plex <plex name>, not attached to volume
<volume name>

my question is in two parts,
one : is there a disk problem? how do i confirm the same?
second : if i change with a new disk how do i go about bringing the disk
in to my volume ?

please help me. this is a real time mission critical site where i cannot
try things out untill i am sure about it.
