[SUMMARY] CAM_ERROR

CAM is a SCSI error type (Common Access Method), not quite sure yet
whether the drive was bad, firmware is wrong, or drive got too hot, but we
are working on it now as a SCSI problem. Uerf actually shows which drive
it was and everything, should have thought of that myself.

Thanks so much to everyone for the fast responses. One of them is below

Use uerf or dia or whatever you prefer to look at your binary error log.

The error that was detected was a SCSI bus reset on bus 0. Usually, the
bus does not get reset unless there are problems with, for instance, a
bus hang. Since you just added a new SCSI drive, if it's on bus 0 (which
is probably an NCR810 or a Symbios chip based board), it's a likely and
maybe even probable cause. But check the error log -- if you don't see
any other errors, it's hard to know for sure.
