dead machine

Hello Managers,

The other day our system (AS4100) suddenly became very unstable
and no logins were permitted and active logins were useless,
could not ping the machine or browse any of it's web pages, use
pop etc. The machine was basically dead but had not actually
crashed as such.

The system

Digital UNIX V4.0D (Rev. 878)

had to be brought down to SUM and it was found that
the file

/etc/auth/system/default.db

was zero length. After restoring the file all was ok.

Has anyone experienced this? What could have caused it?
