Help! A serious problem on Digital Unix 4.0d

Dear Sir:
    We encountered a serious problem on our Alpha 8200. The system down onceevery 24 hours. We read /var/adm/messages file and find the error messages is : ADVFS EXCEPTION
                    Module = bs_delete.c ,Line = 1407
                    del_clean_mcell_list: no primary xtnt record
                    panic(cpu 8): del_clean_mcell_list : no primary xtnt record
                    syncing disks ... done
                    DUMP.prom: dev SCSI 0 1 0 0 0 0 0, block 600000
                    DUMP.prom: dev SCSI 0 1 0 0 0 0 0, blick 600000
I don't know which ADVFS is wrong . The OS is Digital UNIX 4.0D.
Do you have experience on it? Can you help me ? I'll appreciate it
