Printer-headings missing

We have a problem with an OKI393 printer:
On the first page of all reports the heading titles are missing, each
subsequent page is OK. The problem occurs whether you print from Unix or from
the application so I think it must be a Unix setting. We have tried various
setting in the application but to no avail.
The printer is connect via a 'JetDirect' box.
