CAM SCSI error event on Tru64

Hello,
I have a question about an error that I am getting on one of my servers.

We have an Alpha 2100 series with 128 MB ram, running 4.0e. We normally
run without problems.
The last couple of days I have discovered a problem and I am not sure
what to do to find out what is going on.

Tuesday night, our opperator called me and told me that the incremental
tape backup errored our. The error was:
        vdump: unable to write to device </dev/rmt0h>; [5] I/O error
This happened about 1:00 am

>From about the same time I received a ERROR EVENT notification with CAM

SCSI. It is very hard to decipher what I am looking at.
OS Event type 199.
Sequence Number 8

Where do I look to find out what is going on?? What do I do to see more
information??
