Thanks to Alan Pittman, Jim Surlow, Bryan Lavelle, and Mohd Nayle
for their replies.
The consensus was that there is a compatibility problem between the
SWXCR software and the 5.7-4 firmware shipped with 4.0G. I was
told to either upgrade to 5.7-6 or downgrade to 5.6. However, the
only other version I had was 5.3, which I was already running prior
to the upgrade. Juan Fermin from the Compaq Support line told me
I could download the latest version (5.8) from:
ftp://ftp.digital.com/pub/Digital/Alpha/firmware/v5.8/
Also, Bryan Lavelle from Compaq stated that I did need to run the
ECO (EISA Configuration Utility) and that it was proprietary software
that I needed to purchase. If I need it, shouldn't it be supplied with
the hardware/OS? I found a copy in our software library and ran it.
Well, that solved the problem with the messages at during initialization
but now I no longer have a valid bootable disk. I tried booting from
the CD and relabeling but then a fsck trashed the filesystem throwing
everything into 'lost+found'. I intend to just restore are rerun the
upgrade at this point. :(

I have a Mylex DAC960 SWXCR and on initialization after upgrading
the firmware to 5.7, I got the "Dev_table_add: device Mylex DAC960
or bus PCI not found " error. I searched the archives and found to run
ECU (not sure what this is or how to do it) and upgrade the firmware on
the Mylex controller. I figured out how to get into the ARC console and
run the RA200FL to upgrade the firmware with the SWXCRFWP.270
but then on initialization I get the same error.
