INFORMATION BLITZ TITLE:
Systems may hang during restarts after installation of Tru64 UNIX V4.0F
patch kit number 4
PRODUCT NAME(S) IMPACTED: Tru64 UNIX V4.0F P.K. 4
PROBLEM STATEMENT:
After installing Patch Kit 4 restarting the system may cause it to hang.
SOLUTION:
If you have experienced this problem after installing V4.0F Patch Kit 4,
back out "New Patch ID: 417.00", a patched fdi.mod. If you have not
installed Patch Kit 4, do not install patch 417.00.
Comments:
There was a bug introduced in this patch which causes ES40 and possibly
other systems to hang on restart; reverting to the previous fdi.mod allows
these systems to restart without hanging. Patch Kit 5 will not contain the
defective fdi.mod.

Some further notes from me:
If you have built a new target kernel and it won't boot, remember that you
can boot the "generic" /genvmunix kernel to recover.
If you did install the bad patch and didn't make the patches reversible,
you can put back the old fdi.mod by copying it from your installation
media; it should be in /sys/BINARY on your system and in the installation
CDROM media. Or restore it from a backup. Then rebuild your kernel with
the old "fdi.mod" in place.
As the Blitz notes, although this has been seen to impact the ES40 systems
it may impact other systems, so be careful out there..
