LSM - voldiskadd won't add disk

Under DU4.0F, we are playing with LSM, and I'm having problems
adding a disk to a diskgroup. I'm using:

        voldiskadd re6 (system is Alphaserver 1000a with swxcr)

where "re6" is a functional disk that I can disklabel and newfs
with no problem. When I run "voldiskadd" I get:

Initialize a disk
Menu: LogicalStorageManager/Disk/AddDisk

  Initialization of disk device re6 failed.
  Error: voldisk: Device re6: define failed:
        Disk device is offline


but there's nothing wrong with the miserable disk!!!!!!!!
Can anyone advise what it might be doing?
