Solution:

Most all suggested that there is an issue with the kernel and suggested that
I rebuild the kernel. However I was not able to boot into "single user
mode". So I booted from the firmware prompt with the following command:
>>boot -fi /genvmunix -fl s

I then booted into single user mode, rebuilt the kernel and restored /usr.
Finally I was able to boot into multi-user mode; success.
