Tape Device Errors

I have a TLZ09 installed on an alpha 1200 running 5.0. The tape device is
accessible, however is behaving 'strangely'. When I insert the tape, it
takes a while for the activity lights to stop blinking. When I offline the
tape, I often get errors of "place tape online".. when i go to online it, I
get "no medium". Every night for backups, I get an error "place tape on
line" when the tape is already in drive, and I onlined it before leaving.

Below is partially what I get when doing a status. I'm particularly
concerned about the cam_status, scsi_status and scsi_sense_data. What do
those mean?

MTIOCGET ELEMENT CONTENTS
---------------- --------
mt_type MT_ISSCSI
mt_dsreg 0x1
                        DEV_BOM
mt_erreg 0x0 No error or no sense info.
mt_resid 0
mt_fileno 0
mt_blkno 0
DEV_EEI_STATUS
        version 0x2
status 0x3400 Device hardware error (hard error)
        flags 0x1000003

cam_status 0x4 CCB request completed with an err
        scsi_status 0x2 SCSI_STAT_CHECK_CONDITION
        scsi_sense_data not valid

Does anyone have any ideas??
