My first thought would be that someone has a sense of humour and tradition. In
the RSX-11D and IAS operating system the error was (and assume is) defined as

        IE.NFW .IOER. -69,<Path lost to partner> ; This must be odd

The error code was used to flag when a connection was lost on a communications
circuit. Outside of the source code, I don't think it was ever documented.
