problems with SEAGATE disk installation on ALPHA station

Dear managers,
I should help one of our users to install on alpha 200 station running
DU 3.2a
outer Seagate Barrakuda 4 Family ST15150 N/ND drive. The disk was installed
by the
techician from the dealer firm. I can see it at monitor prompt (show dev) as
DK100. After reconfiguring the kernel it appears as rz1.
All my attempts to initialize the disk were unsuccesful.

Question:
Is it hardware problem with the disk, or I do something (or all :-)) wrong.
Please help.
